PAY RATE: $18.64 Hourly (Pay is negotiable based on experience)

Summary:

Performs targeted case coordination services in accordance with KY Administrative Regulation. Responsible for conducting a comprehensive assessment and periodic reassessment of participant's strengths and needs, then assists them to gain access to identified medical, social, educational, and other services needed.

Essential Job Functions:

  • Responsible for arranging for service delivery from the participant's, the participant’s legal guardian, or the participants’ custodial parent's chosen provider to ensure access to required services; (c)Facilitating access to needed services by explaining the need and importance of services in relation to the participant's condition; (d)Facilitating access, quality, and delivery of necessary services.
  • Assists participants in gaining access to needed medical, social, educational, or other needed services and supports, including: (a)Assessment of the client's medical, social, and functional status and identification of the participant's strengths and needs.
  • Responsible for preparation and maintenance of case record documentation to include care plans, forms, reports, and narratives as appropriate.

Qualifications

Education:

Bachelor’s degree in one of the following fields:
Psychology, Sociology, Social Work, Family Studies, Human Services, Counseling, Nursing, Behavioral Analysis, Public Health, Special Education, Gerontology, Recreational Therapy, Education, Occupational Therapy, Physical Therapy, Speech-Language Pathology, Rehabilitation Counseling, or Faith-Based Education; or be a certified Alcohol and Drug Counselor who holds a Bachelor of Arts or Bachelor of Science degree.

Years of Experience:

2 or more years of full-time employment experience working directly with adults or individuals under the age of 21 in a human services setting.

Certifications and Licensure:

Possess a Valid Kentucky Driver's License and Maintain a Safe Driving Record Required

CAP Driving Certification* (or ability to obtain within first 90 days) Required

*In order to drive personal vehicle on CAP business, must obtain and maintain a policy of motor vehicle insurance coverage on privately-owned vehicle with insurance coverage amounts of $50,000 bodily injury per person per accident; $100,000 bodily injury for all bodily injuries per accident; and $50,000 property damage per accident.
